Patient Registration Form

To make your visit go as smooth as possible, please open and fill-in the Patient Registration Form and bring it with you for your appointment.

Cancellations

If you are unable to keep your appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient. A charge may be applied for missed appointments unless you call and cancel or reschedule your appointment.

Health Insurance Plans

Presently we participate with:

  • CareFirst of Maryland BlueCross Blue Shield PPO, PPN, POS, indemnity
    (Note: Blue Choice (HMO) is not a participating insurance)
  • Federal Care First BCBS
  • Medicare non-HMO (we will accept Medicare Advantage PPO plans)
  • Railroad Medicare
  • Cigna (except Dr. Jan Brown)
  • Aetna (except Dr. Jan Brown)
  • United Health Care (except Dr. Jan Brown)

If you have a Medicare replacement plan, check with our staff for details. Because of the rapid changes in healthcare, changes occur frequently. Feel free to ask our staff if your plan is now included.

Applicable copays, coinsurance and deductibles will be collected on the day you are seen if you have participating insurance or network insurance. If no insurance or out-of-network insurance, we collect our charge on the day of service. Your insurance may then offer some reimbursement based upon your out-of-network coverage. If non-participating, the full charge will be collected and your insurance will reimburse you directly based upon their coverage.

Filing of claim forms to your insurance is part of our service to you. Our only exception-HMO plans.

If your plan requires a referral, we must have it when you are seen.

Financial Policy

Just as we are committed to providing excellent care, we are also concerned with making it affordable to you. By reducing costly billing procedures and requesting payment at the time services are rendered, we can keep our costs down and pass the savings on to our patients. For your convenience, we accept MasterCard, VISA, Discover, personal checks and cash.

Emergencies

We will make every effort to see emergencies in the office as soon as possible. When the office is closed you can cancel appointments or leave messages regarding billing, pharmacy or medical problems, which we will respond to on the next business day. For urgent problems only, our answering service can be dialed directly at (410) 787-4300. They will contact us for true emergencies.

In any life threatening emergency, such as serious drug reaction,
go to the nearest emergency room at once!
